I release the use of photos and/or videos of my student-athlete participating in Next Level Spartans activities and events.

By entering my name (electronic signature) below and submitting this registration form, I certify I opened, read, and agree to the terms of participation explained in the NL SPARTANS RELEASE OF LIABILITY FOR MINOR PARTICIPANTS.

I agree to submit the full registration fees by July 1, 2025, and provide the required documentation by the stated deadlines or face removal from the roster. Equipment will not be issued if fees are not fully paid and the required forms have not been turned in by the deadline. If I selected a payment plan, I understand it is my responsibility to return to the site to complete the remaining fees. Registration fees are not refundable or transferrable after the first day of practice, July 8, 2025.

I understand game day participation is not guaranteed regardless of fees paid or attendance. Game day participation is earned based on student-athlete readiness as determined by NL Spartans coaches.

I understand that only coaches and team managers with badges are allowed on the practice and game field and agree to remain in designated spectator areas and off the tracks.

I understand there is a 48-hour post-game waiting period required before speaking with coaches about the game. I agree to display great sportsmanship at all times.
